There are one in four people over 65 living with two or more chronic illnesses. Chronic conditions include diabetes, heart disease, epilepsy, respiratory disease, Alzheimer’s disease and dementia. The ESRI in 2009 estimated that the number of patients over 65 will triple over the next 30 years. Chronic diseases will increase proportionally indicating the need to accelerate progress towards person-centred care. Prevalence of multiple chronic illnesses has led to higher risks leading to an increase in the number of hospitalisations resulting in fragmented care for the patient. Our service includes medical monitoring and assistance in the comfort of your own home reducing need to attend inpatient settings.

We provide Palliative Care for people in the comfort of their own home. Our experienced staff provides the highest level of holistic care in a compassionate and dignified manner promoting independence where appropriate. We provide assistance with day to day tasks and where required assistance repositioning for comfort. Our Nurses come to your home to administer injections and medication. We provide person centred care in the comfort of the home setting. Family input is facilitated and encouraged. When the time comes we deliver end of life care in a peaceful dignified manner in accordance with our service users and their families’ wishes.

According to the ESRI Ireland had the highest birth rate of any EU state in 2013. We understand that giving birth is a life changing experience. Our postnatal care package provides holistic care management for both mother and baby on discharge home from hospital. Our Midwives carry out a daily postnatal assessment on Mum and daily neonatal assessment on baby. We provide assessment and treatment for associated wounds ( Caesarean section wound, episiotomy, 3rd/4th degree tears). We complete the PKU (heel prick) test for newborns. Our experienced staff provide breastfeeding assistance. No package requirement is too big or small. Our counselling service is available to those who require it.

We facilitate homecare packages at all levels for children with life-limiting conditions. We cater for children requiring home nursing and health care assistance. Children are surviving longer with the availability of ever advancing treatments. In order to be discharged from acute hospital care the increased need for homecare packages is evident. We provide staff to cater for tracheostomy and home ventilation, home dialysis, enteral feeding, seizure management, wound management, oxygen assessment and administration and vaccination administration. Where appropriate we provide assistance with activities of daily living. Our packages are provided in the comfort of our service users own home at a time to suit the requirements of the child and family.

Often further medical treatment is required after discharge home from hospital. Our experienced Nurses provide this treatment in the comfort of our service users home. Our Nurses take blood specimens for testing and these samples are delivered to the hospital that day for analysis. We administer injections as required. We provide stoma care for service users with newly formed and long term stomas. We provide bowel and bladder care including catheter care, urinalysis and suppository/enema administration. We assist in administering enteral feeding. We care for patients with various IV lines and their requirements such as IV antibiotics and administration of Total Parenteral Nutrition (TPN). We provide blood glucose monitoring. We also provide ongoing education to patients and loved ones.

We provide a wide range of services which assist and compliment rehabilitation from acute or chronic illnesses. Our Multi Disciplinary Team work together to ensure the best possible recovery outcome is achieved. Our Occupational Therapist assesses the family home implements structural changes and recommends necessary equipment to assist rehabilitation. Our Physiotherapist assesses mobility and works with the service user to devise an exercise programme. Our Health Care Assistants assist with the completion of these exercises and provide assistance with activities of daily living. Our Nurses provide post-operative wound care and assess the progress of each service user on an ongoing basis. We liaise closely as a team to ensure the best possible outcome for each of our service users.

We provide excellent rehabilitation assistance to our service users who have suffered a stroke. We assess each service user as an individual. We strive to promote independence through our person centred approach. We encourage close involvement of family members and loved ones. Our experienced care staff provide any assistance required particular to each service user. Such tasks include activities of daily living such as bathing dressing eating and drinking, identifying correct nutritional requirements toileting and communication. Each service users progress is assessed on an ongoing basis. Our Nursing and Care staff liaise closely with our Dietician, Speech and Language Therapist, Occupational Therapist and Physiotherapist to ensure the optimum level of recovery for our service users.

Our Multi Disciplinary Team work closely together to assist in facilitating independent living in the comfort of your own home. As well as providing assistance with day to day tasks such as bathing, dressing, toileting, mobility, eating and drinking we also provide various levels of assistance with domestic care. Our experienced staff provide care in a kind, friendly and trustworthy manner ensuring family and loved ones can spend more precious time together.
